About our department
The Extensive Care Service are a team of advanced clinical practitioners, enhanced clinical practitioners and assistance practitioners who work in the community. The service provides a proactive medical management approach for patients with a long-term condition with the aim of preventing unnecessary hospital admissions, offering assessments and medical optimisation so patients can manage their condition more effectively at home. We also support patients who have had a recent attendance to A&E or an admission to the hospital.
The team are supported by a consultant cardiologist and a GP through weekly Multiple Disciplinary Team (MDT) and offer a medical guidance to more complex patients with multiple co-morbidities.
The team cover Tameside and Glossop locality, working together with other health and social services in each neighbourhood and Tameside Hospital. All care is delivered in your own home, and we aim to maximise your health and wellbeing by educating and supporting you, your family or carer to manage your condition with confidence.
The team have specialist knowledge of long-term conditions including frailty, respiratory and cardiac conditions. Where necessary, the team will provide clinical assessments, investigations, diagnostics, treatment and referrals to other health professionals to provide holistic care.

- Open access to the service if you were to become unwell or require further support.
You can be referred to the service by your GP, community health or social care professional, or by the health care professional at the hospital when you are discharged home. You can contact us yourself using the referral form.
Following your referral, a member of the team will contact you by telephone to discuss your care needs and arrange a clinic or home visit if necessary. During the visit, you will receive a comprehensive assessment to identify your care or treatment needs.
Extensive Care Service criteria
- Over the age of 18 years old
- Long-term condition, or experiencing reoccurring falls, delirium, constipation, polypharmacy immobility
- Live in the Tameside or Glossop area
- Registered with a GP in Tameside or Glossop
