About our department
We are a multidisciplinary team, offering rehabilitation to adults whom have a neurological condition such as stroke, multiple sclerosis, Parkinson’s disease or head injury.
We are an integrated team made up of physiotherapists, occupational therapists, specialist nurses, speech and language therapists, clinical psychologists and support staff. We offer a co-ordinated approach to rehabilitation, tailored to meet each individual’s goals and aims.
We will work with you and your family or carer to help you to understand your condition and symptoms. We will assess your abilities and address your difficulties with treatment programmes and we will support you to develop strategies to enable you to live your life with a long term condition.
Our service is delivered by staff with specialist skills and knowledge, in the management of neurological problems. This means that the team can support and advise you on many aspects of living with this kind of difficulty.
Your GP, consultant, specialist nurse or social worker can make a referral to our team.
You will be contacted by a clinician to discuss the reason for your referral. You will be asked some questions to help us to get the correct people to visit you. At this point, we may be able to offer you some advice on the telephone.
Our team members will assess your abilities and difficulties. This will help you and us to prioritise your goals and assist us in developing your individual treatment plan. Your treatment plan may consist of one to one home visits or group exercise and education sessions, alongside activities for you to practise.
You will be advised on activities to practise in between sessions. This forms part of the rehabilitation process and is just as important as the things you do whilst our team members are with you.
